Action Together Home from Hospital Fund

We are pleased to announce that we have successfully secured grant funding from the Action Together Home from Hospital Fund.

This funding will enable our Wellbeing and Supportive Care team to deliver a six-month pilot of Day Respite, designed to support palliative patients and their carers.

This pilot aims to:

  • Provide much-needed respite for patients and carers
  • Reduce isolation and promote social connection
  • Support patients to better manage the impact of their condition through practical techniques, such as relaxation strategies to help reduce pain, stress and anxiety, advice on managing sleep difficulties, and general supportive guidance to enhance quality of life
  • Improve patients’ physical, mental and emotional wellbeing, while building confidence
  • Enable patients to remain as independent and comfortable as possible, helping to reduce the risk of hospital admissions

Planning for the pilot is now underway and we look forward to sharing updates as the project develops!

Please note: This is not a service we currently offer self-referrals for. Our Wellbeing and Supportive Care Team will be receiving referrals from specified health and social care professionals for this pilot.
