What is Hospice at Home?

  • Our team supports patients at the end of life to be cared for in their preferred place of care such as their own home, rather than in the Hospice. 
  • Our Hospice at Home Team work alongside our external partners, including the District Nursing Team, home care providers, and Specialist Palliative Care Nurses, to ensure patients receive coordinated and comprehensive support 
  • Our Hospice at Home nursing team consists of experienced Hospice nurses who will visit patients, deemed to be in their last two weeks of life, providing a flexible and responsive service in the patient’s own home. 

What are the aims?

  • Provide patients and their loved ones with the appropriate care to ensure the patient has the support they need to remain in their own home  
  • Prevent any unnecessary admissions to hospital  
  • Work alongside District Nurses and Specialist Palliative Care Nurses to support them in caring for their patients, providing an added layer of care, comfort, and reassurance during a difficult time 

How does it work?

  • Patients have to be referred by a Health Care Professional  
  • Hospice at Home patients, along with their loved ones and carers, will be provided with a dedicated mobile number to contact the team directly with any questions or concerns about care 
  • The Hospice at Home Team is available between the hours of 8am to 9pm
