Specialist Palliative Community Care

Our Community Service aims to provide care to patients and families in their own homes.

We strive to deliver high quality community integrated palliative and end of life care service for patients within Heywood, Middleton and Rochdale area.  We aim to focus on quality of life, promoting the physical, psychological and spiritual wellbeing of those individuals deemed to be in the last 12 months of life.

Medical Support
The clinical lead for the community service is a specialist palliative care doctor with several years' experience. Medical outpatient appointments and domiciliary visits can be arranged via your GP, Hospital specialist team or Community Specialist Palliative Care Nurse.
Specialist Nursing Team

The nurses offer specialist support for patients with advanced life limiting diseases and work closely with the patient's GP and primary care team by;

  • Supporting the patient in their preferred place of care.
  • Offering advice and support to the family/carer in relation to the patient's illness.
  • Advising and supporting on symptom control issues.

Our Specialist Nursing Team provides support 7 days a week with extended working hours between 8am - 10pm weekdays and 8am - 4pm on weekends and bank holidays.

Hospice at Home

Our Hospice at Home team delivers skilled nursing care to patients at the end of life and provides support to their family and carers in their own home. The team work closely with current services including, District Nurses, to provide an additional element of support at this difficult time.

Night Sitting Service

Skilled care staff are able to provide night sits to allow a carer respite and rest. This means patients can continue to be cared for in their preferred place, and the comfort of their own home.

The primary aim of our Physiotherapy service is to support the patient to achieve their maximum potential function within their own limits, in turn, promoting independence and dignity and improving the patients quality of life.

24 Hour Advice Line

Advice and support is available 24 hours a day for both healthcare professionals and patients/families. With our 24-hour Advice Line you can speak directly to a qualified and experienced member of the nursing team.

Volunteer Support

Community Volunteers support patients and families in their own homes, providing a valuable few hours of companionship with a patient, allowing families and carers a well-earned break. Due to the ongoing pandemic, we are currently offering a telephone befriending service. This involves having a weekly chat with the patient who may be feeling lonely or isolated. Counselling and Spiritual Care

Psychological, emotional and spiritual support is available to patients and families from the Psychological and Supportive Care Team.

During the coronavirus pandemic, counselling has mostly been provided to patients and relatives by telephone or video. As things return to normal, we hope to offer more face-to-face counselling sessions, both at the hospice and at other venues in the community. Home visits are available for house-bound patients.
