Photography and Video permissions

I agree for my and/or the child’s (for whom I have parental responsibility) words, information, story, photographs and video – as set out overleaf – to be used by Springhill Hospice in printed, online or electronic publications, which may be shared publicly. Examples of these materials include, but are not limited to: posters, leaflets, brochures, adverts, emails, newsletters, blogs, web pages, social media posts, presentations, speeches, press releases or other materials, unless contradicted by my exceptions overleaf

I understand that I and/or my child do not own the copyright or have any rights of ownership or other claims over the photographs, video recordings, quotes and information about me or the child for whom I have parental responsibility

I understand Springhill Hospice will store and use my photographs, videos recordings, quotes and information securely and in line with legislation including the General Data Protection Regulation

I understand that any information about myself and/or the child for whom I have parental responsibility will only be used for the stated purposes of this form. Springhill Hospice will not pass my images, details or story onto any other agency for use beyond the above purposes without gaining prior consent. Springhill Hospice cannot guarantee or be held liable for use of this information or images by a third party

I understand that I can withdraw my consent at any time, or update my communication preferences by contacting Springhill Hospice. If I withdraw my consent, Springhill Hospice will no longer use any information I have provided previously. I understand that any published materials that already includes my information cannot be recalled

I understand that my consent applies for five years from the date below. After this date, photographs, video recordings and quotes will be catalogued and will only be used in historical, anniversary and special publications as deemed appropriate by the Hospice

Content published online or via social media may remain publicly accessible indefinitely.

In exceptional cases – for example, the use of historic images – the Head of Income Generation & Marketing may authorise content for use of images outside these guidelines.
